Youth Registration Form

We welcome youth ages 14-18. Campers must be 14 years of age by July 31, and not older than 18 when coming to camp. The cost of camp $285.00 per person, which includes lodging for 5 nights, 15 meals, all activities, T-shirt, water bottle, wristband, and photo DVD.

Please return this form no later than June 20, 2008, along with a $25.00 non-refundable deposit. The remaining balance of $260.00 is due no later than July 1, 2008. Fill out a separate form for each youth wanting to attend. Please fill out all applicable fields to the best of your ability.

Part II: Payment

A $25.00 non-refundable deposit is due with your completed registration form. The form and deposit is due no later than June 20, 2008. The balance of $260.00 is due no later than July 1, 2008. Make checks payable to Lion and Lamb Ministries, OR you may pay by credit card with this secure form.

If your registration form is received after June 20, there is no guarantee that you will receive any T-shirts, water bottles or wristbands.

Part III: Arrival and Departure

I am responsible for my own transportation to and from Oakridge Christian Camp in Anadarko, OK. (Maps will be in your confirmation packets.) I will arrive not earlier than 2:00 pm on Sunday, July 13th, 2008 at Oakridge. Camp begins at 3:00 PM. Camp ends on Friday, July 18th between noon and 2pm.

I am requesting assistance for housing and transportation to and from Oakridge Camp through a host family. (Please contact the office and speak with Amy about your needs.)

Part IV: Submission

IMPORTANT: Please remember to send the completed Parental Permission Form/Youth Contract, the completed Oakridge Camp Minor Release Form, photocopies of your insurance policy cards (if applicable), and your payment (if paying by check) after you submit your registration.
